Purdue University Student Services (PUSH) provides allergy injection services for all eligible students who have a referral from their allergist. Prior to receiving allergy injections at PUSH, patients will need to meet with their allergist and complete an allergy documentation packet which includes the forms listed below.

Once these forms are completed and submitted to PUSH, patients can call 765-494-1700 to schedule with our Allergy and Immunization Services department.
